Important Note:

New Students must complete a School Registration Form when they come to their first class. Returning Students need not complete a Registration Form unless there has been a change of information. In order for students to take a class, LDT MUST have a Registration Form on file with the liability waiver signed by the student (if over age 18) or a parent/guardian.
